I have a problem with the form. It is as a PDF file and I do not have the software to edit it. Therefore, I will have to complete it by hand and since I cannot read my own writing you may have a problem especially as you will have to transcribe it.

Would it be acceptable if I reproduce the form in WORD, complete it and send it with my prints. In fact do you want a PMed copy so that it avoids the transcription?

Others may have a similar problem, hence the reason for putting it in this thread rather than PM.

current acrobat reader allows you to update forms in PDF's providing you haven't switched that capability off in options. (I think)

OR

you can save it as TXT which will save what you have typed in as well.

I think whichever way you feel happiest with will be fine Mike. The ones I've received so far have mostly been hand written on a print out.
Why not email it to me and I'll add it to your prints when they arrive.

The same is fine for anyone else who needs to do it this way.
